Instructions:

To nominate someone for a Society For Biomaterials 2009 Award, please follow the procedure outlined below:

  1. Log in above using your username (email address) and password (SFB Membership number). If you do not know your SFB membership number, please browse to http://www.biomaterials.org/login/login.cfm
    If you are not a member of the Society For Biomaterials, please click “I do not have a login and would like to register” After entering your information you will be given a log in number that will allow you to return to your nominations for further editing or uploading.
  2. On the resultant “New Nomination” page, enter the nominee’s full name, and select the award for which you would like to nominate them.
  3. The following page is the nominations page for this individual. Please bookmark the page, or copy the URL for further editing at a later date.
  4. On this nomination page, you will be asked to upload several documents, and provide demographic information for the nominee:
    1. To add demographic information, click the “Edit nominee’s Info” link.
    2. Be sure to upload the candidate’s CV.
    3. Supporting Letters of Recommendation: each nomination must be accompanied by at least three supporting letter s of recommendation. One should be from the nominator, and there should be at least two others.
    4. Other supporting documents: Each award has specific requirements for other documentation which are listed on the awards description page. These include:
      1. Manuscript
      2. Biosketch
      3. Product literature
      4. Certification of Status
    5. You may upload a photo of the candidate as well, if one is available, but this is not required.
  5. BEFORE LOGGING OUT: Please note the URL of the page for future editing.
  6. Once you have entered the contact information for the nominee, and completed uploading the required documentation, please be sure to log out to ensure that your changes have been saved.
